REGISTRATION PROCESS

To ensure we can provide you with the services you need, we ask that you first complete the registration process by submitting the required information and paperwork online.
Once we receive your submission, our team will carefully review your details to determine your eligibility. If additional information is needed, we will contact you directly. Once eligibility is confirmed, we will reach out to schedule an appointment so you can access the services you qualify for.

VETERAN'S REQUIREMENTS AND QUALIFICATIONS

Must provide a DD-214 (Must have six (6) months Active Duty service, If under 6 months service, may qualify if under a Medical Discharge).

Must provide a valid VA picture I.D. card or valid state issued I.D. or driver’s license.

If requesting financial or rental assistance. please provide copy of current lease (must be in your name and active).

All Veteran's must Sign a release of information from the VA for housing and financial discloser.

Veteran must comply with the above requirements, or they may be determined ineligible for services.
